9/17/2026 severe green ocular discharge abrasions on both front elbows wound on right face cheek placed in S kennel per KT 9/17/2026 09/17/26 17:15 BAR BCS 6/9 MM-pink, CRT<2sec, grade 2 dental tartar Eyes-entropion OU, lower lids EN-WNL H/L-WNL HR 70, RR 35 Derm-4-5 inch irregularly shaped epidermal mass on right cheek with serosanguinous drainage. Masses on both elbows each of them also with draining tracts. M/S-WNL Neuro-WNL Urogenital-WNL Assessment: adult female with epidermal masses on face and both elbows. all with draining tracts. DDX: neoplasia, furunculosis/deep infection, cocci, open. Prognosis poor to guarded. Plan: start cephalexin 750mg po bid x 7 days. Dog needs follow up with family vet for biopsies and long term treatment. Due to extent of lesions, dog will not be undergoing anesthesia or surgery at MCACC. Dog is not available for PUFA or foster. Placing on portal urgent medical NHO. If no outlet by COB on 09/24/26, is at risk for humane euthanasia on 09/25/26. 9/17/2026 Nobivac 3-Rabies, manufactured by Zoetis, Serial number: 859498 administered SQ, R lateral hip, by _BJ__, supervising veterinarian ST. Nobivac Canine 1-DAPPv, manufactured by Merck, Serial number: 02121B49B administered SQ, R lateral shoulder by BJ___. Nobivac Intra-Trac3, manufactured by Merck, Serial number: 00541534B administered IN by BJ___. Bravecto (fluralaner) for dogs >44-88lb (1000mg) 1 tube of topical solution applied in multiple spots between shoulder blades and along back by _BJ__. Reapply after three months. Female: No tattoo or scar seen with adult mammary and vulvar development, consistent with unaltered female. Confirmed by: __CLINIC__ Mylar placed on dog.
